Common Symptoms of Perimenopause

Technically, ‘menopause’ is characterized by absence of menstruation for 12 months in a row. In simpler words it means when the periods have stopped. This happens as the ovaries stop producing estrogen and other female reproductive hormones. We are all quite aware of this phenomenon. However, the concept of ‘pre-menopause’ and ‘perimenopause’ still remain obscure. Let’s try and understand them better.

The term pre-menopause literally means ‘before menopause’ while perimenopause is defined as ‘around menopause’. However, in broader sense, perimenopause is the commonly used term to denote both (time before menopause and just around menopause). It basically refers to the time when (menopausal) hormonal changes have already begun while the menstruation is still on. This is also known as menopausal transition, and often occurs between the ages 45 and 55. The perimenopause is usually for 7 years but sometimes has been noted for as long as 14 years.

The perimenopause impacts each woman differently. Some women might not experience any discomforting menopausal symptoms, in fact are relieved not to worry about painful or disconcerting periods. While for others the menopausal transition can mean various discomfited symptoms like hot flashes, irritability, loss of libido, depression, changes in shape and body composition, etc.

The common symptoms during perimenopause are:

  • Irregular periods – due to unpredictable ovulation the length between the menses might vary drastically, sometimes even skipping a month or more. This could also affect one’s flow.

  • Hot flashes – Sudden feeling of warmth in the body with profuse sweating and occasional skin reddening. Often treatment for hot flashes is not required, but relief from symptoms improves the quality of life.

  • Sleep troubles – This could be either due to hot flashes or due to hormonal changes.

  • Vaginal and Bladder problems – The reducing oestrogen levels can make the vaginal tissues lose lubrication and elasticity, leading to painful intercourse. Low estrogen may also cause vulnerability to urinary or vaginal infections. Urgency to urinate and urinary incontinence can be caused due to loss of tissue tone.

  • Changes in libido – The hormonal changes can cause changes in arousal and desire as well. Sometimes changes in sexual function could be a result of painful intercourse as mentioned earlier.

  • Reduced bone density – Estrogen levels in body are directly proportional to calcium absorption. Also progesterone is known to stimulate calcitonin hormone which plays an important role in calcium synthesis. The reduction in progesterone and oestrogen levels increase risk of osteoporosis i.e. weak bones which lead to joint and muscle pain.

  • Mood Swings – Changes in mood, irritability or depression are common during menopausal transition. These could be resultant of sleep disruption or hormonal changes during perimenopause.

  • Changes in other body functions – Disruptive bowel moments, sudden change in weight (usually weight gain), changes in body shape and composition, loss of enthusiasm and energy, dyslipidemia (increase in LDL i.e. bad cholesterol and decrease in HDL i.e. good cholesterol) leading to increased risk of heart disease are some changes noticed in body
